The County Remittance Form is used by counties to report the Multi-County Assessments (CAMA), Charter School Levy Payments, Marriage License Fees, and the Children Defense Trust Fund Payments to the Utah Office of State Treasurer. Monthly collections should be reported on this form and remitted to the Utah Office of State Treasurer at the below address by the 10th of each month:
